1. Rental Agreement

By submitting this form, you agree to rent medical equipment from CuraScript Healthcare Solutions (CuraEquip) under the following terms.

2. Payment Terms

Rental fees are due upon delivery unless other arrangements have been made. We accept cash, mobile money, and bank transfers.

3. Equipment Condition

All equipment is provided in good working condition. You agree to return the equipment in the same condition, normal wear and tear excepted.

4. Liability

The renter is responsible for any damage to or loss of the equipment during the rental period. Additional charges may apply for damaged or lost equipment.

5. Delivery and Pickup

Delivery and pickup are included in the rental fee within Kampala. Additional charges may apply for locations outside Kampala.

6. Cancellation Policy

Cancellations must be made at least 24 hours before the scheduled delivery time to avoid charges.

7. Maintenance and Support

We provide maintenance and support throughout the rental period. Please contact us immediately if you experience any issues with the equipment.

8. Privacy

Your personal and medical information will be kept confidential and used only for the purpose of providing our services.
